Breathlessness after travel

Pulmonary · Independent CANRA preparation

Original fictional educational scenario, not personal medical advice or an official examination item. Independent clinical educator review is pending.

Clinical study scenario

A 49-year-old develops sudden breathlessness and pain on inspiration after a long flight. One calf is swollen. They have nearly fainted and ask for a breathing exercise.

Connect mechanism to the presentation

A leg-vein clot can travel to pulmonary vessels. The presentation is concerning for a pulmonary embolism.

Explain the next action

Sudden symptoms with near-syncope warrant emergency assessment; state the suspected thromboembolic risk in handover.

When the situation changes

A single oxygen saturation reading is normal. Does that safely exclude the concern?

No. Do not use an isolated reassuring measurement to dismiss the symptom pattern or cancel urgent assessment.

Safety and follow-up

After definitive evaluation, follow the treating team’s plan and coordinate medication and bleeding-risk review where relevant.

Practise actively

Choose an answer before viewing the reasoning, compare the alternatives, and explain your handover.
